Police Record Check

All employees & volunteers at The Oxford School are required to provide a Police Record Check. Police Record Checks must be completed once every five years, each subsequent year an Offence Declaration form must be completed. If an Offence Declaration lapses a new Police Record Check will be required.

To complete your Police Record Check, obtain a signed “Request for Police Record Check” form from TOS administration, during regular business hours (Monday to Friday 9:00AM - 3:30PM). After obtaining a sign Request for Police Record Check, the check can be submitted using one of the below options. Police checks can take anywhere from two to six weeks to be processed.

 

Residents Outside of Woodstock

Submit your police record check at the OPP Oxford Detachment, located at 90 Concession Street East in Tillsonburg.
Record Check submissions can be completed Monday to Friday from 8:30am to 4:30pm.

Please bring the following with you:

  • Your completed Police Vulnerable Sector Check Form (Click button below to download the form)

  • A “Request for Police Record Check” signed by the Head of School, requesting the Vulnerable Sector check, from TOS administration

  • 2 pieces of identification, including 1 photo identification (see below for a list of identification options)

  • No payment required for volunteers


Woodstock Residents

Submit your police record check at the Woodstock Police Service Station, located at 615 Dundas St. in Woodstock.
Record Check submissions and pick-ups can be completed 24 hours a day 7 days a week.

Please bring the following with you:

  • A “Request for Police Record Check” signed by the Head of School, requesting the Vulnerable Sector check, from TOS administration

  • 2 pieces of identification, including 1 photo identification (see below for a list of identification options)

  • A form of payment for the $15 fee (cash or debit only)


Identification Options

ACCEPTABLE FORMS OF PHOTO IDENTIFICATION:

  • Passport

  • Driver’s License

  • Military Employment Card

  • Canadian Citizenship Card

  • First Nations Status Card

  • Permanent Resident Card

  • CNIB Card

  • BYID - Bring Your Identification, issued by the LCBO

  • Ontario Photo ID Card, issued by the MTO

ACCEPTABLE FORMS OF NON-PHOTO IDENTIFICATION:

  • Birth Certificates

  • Baptismal Certificate

  • Hunting License

  • Canadian Blood Donor Card

  • Immigrations Papers

Ontario Health Cards and Social Insurance Number (SIN) Cards are not permitted as acceptable forms of identification.
